Q: When do we submit
our data?
A: You will submit
your data (if you choose) at the end of the 18 month project, so around
December 2001. At that time we will provide you with instructions on how
to extract and submit your data, and what to include or exclude. Some people
have already sent their data, which is fine, but we prefer that you hold
onto it until the end of the project. Data

Q: Should we send
summaries of our data?
A: No – don’t send
your summaries to us. We will eventually want the hard data; the information
you entered into the database. Some people have already sent summaries
of their pre-data which is fine; we will keep that information with the
organization’s files. Data

Q: Can we decide not
to send you our data?
A: Yes. Data submission
is not required. However, the more data we receive, the more meaningful
our evaluation and subsequent feedback to participants will be. Hospitals
that submit all forms, data, and evaluation will receive a $50 rebate for
the cost of the materials.
Q: Is there an internal
tracking number or do we have to do anything to identify the data as ours?
A: When you submit your
data, you will take out all information that would allow us to track records
to a specific patient. You will submit your data in a database named after
your POP ID number. This is sufficient for us. Data will not be identified
by hospital, but may be identified by hospital characteristic (i.e., size,
location, etc). Data
