How do I register for the program?
If you are a UW-Madison faculty or staff:
- Download this application and fill out the questions.
- Once completed, print out application, attach all documents, and mail to:
Office of Child Care & Family Resources
c/o Kids Kare Program (Faculty/Staff)
611 Eagle Heights, Room 148
Madison, WI 53705
- After processing, you will recieve a Chicken Soup enrollment packet to be completed and returned to Chicken Soup, LLC.
